Answer the Call for Interdisciplinary Care Team Meetings October 15, 2024 The Care Coordination team at Samaritan Health Plans may invite you to participate in an Interdisciplinary Care Team (ICT) meeting. These meetings seek to identify any gaps in care and strategize ways to improve care coordination within IHN-CCO member care teams. Your input at these meetings is vital to help us identify additional services and supports for your patients. During the meetings, you will: Share relevant information with all providers on the care team. Collaborate with the team to create a holistic view of your patient’s health status. Recommend clinical interventions tailored to your patient’s needs. In accordance with the revised Oregon Administrative Rules, IHN-CCO members identified as requiring Long-Term Services and Supports or those with Special Health Care Needs must have ICT meetings to include themselves and their care team. These meetings are convened twice per month and within 14 days (about 2 weeks) after transitioning to a new care setting or level of care. These meetings serve as a valuable platform for IHN-CCO members to engage with their care team, share feedback about their care experience and self-report progress toward their Care Plan. Our clinical care managers will schedule ICT meetings, and we ask that you (or a representative from your office) join us as an important participant.
