Select the plan you are enrolled in to ensure you find a provider that is covered by your plan. If your employer sponsors your health plan, select Employer Group. If you are on the Oregon Health Plan, select InterCommunity Health Network CCO. If you are looking for Samaritan Medicare plan providers, select Samaritan Medicare Advantage. If you are an employee of Samaritan Health Services, select Samaritan Choice.

In general, beneficiaries must use network providers to access their medical benefits, except in non-routine circumstances requiring urgent or emergent care. A prior authorization is required to see a non-contracted provider. If you don’t have appropriate authorization from our plan before you see a provider that is not part of our plan’s network, you may have to pay for the services he/she provided you with.
