As a member of Samaritan Advantage Health Plans, you have an over-the-counter (OTC) benefit every quarter. This benefit allows you to get OTC products you may need. Be sure to use your Samaritan Advantage Health Plans Benefits Mastercard when placing your order. Simply order online, mail your completed order form or call 800-752-3730 (TTY: 711). Your order will be shipped directly to your door.

If you purchased an over-the-counter item before you received your benefits card, your card did not work, or an item was not covered when you think it should have been, you can request reimbursement by completing this form.

Do you need to replace a benefit card that was lost, stolen or not received?
Go to MyHealthPlan.samhealth.org, log in and then click on the button that says, “Benefit Card Login”. Once in the portal, you can report the first card sent as lost or stolen and a new card will be mailed to you. Another option is to contact Customer Service and request a new card be mailed.
