Healthy Habits Incentive

For 2025, Samaritan employees who are Choice medical plan subscribers (the employee who has the health plan premium withheld from their paycheck) can receive $150 by earning a total of 20 points for completing a variety of health-related activities. Options include a routine physical exam or preventative screening, dental exam, flu shot, online courses and financial wellness events.

Track and manage your points on the MyWellness portal (access from MyHealthPlan).

Options for 2025	How to Earn Points	Points Awarded
Preventive Routine Physical Exam or Screening	How to Earn Points Complete up to two of the following: annual routine physical exam, mammogram, colorectal screening, prostate exam, or pap smear. Your points will be uploaded to the MyWellness portal within 90 days after your exam.	Points Awarded 8 points/exam; maximum of 16 points allowed.
Preventive Dental Exam	How to Earn Points Self-report your dental exam through the MyWellness portal.	Points Awarded 5 points.
Flu Shot	How to Earn Points Self-report your flu shot through the MyWellness portal. If you receive your flu shot from a provider, your points will be automatically uploaded to the MyWellness portal within 90 days after your flu shot visit.	Points Awarded 2 points.
Online Course	How to Earn Points Through the MyWellness portal, complete online health and wellness courses. Ten unique courses are available.	Points Awarded 2 points/course; maximum of 12 points allowed.
Financial Wellness Events	How to Earn Points Complete any of the SHS sponsored Financial Wellness events listed below. Watch for email announcements or contact Samaritan Human Resources regarding available event dates and times. Points for these events will be uploaded to the MyWellness portal on a quarterly basis. Principal: • Attend a one-on-one Retire Secure meeting. • Set up beneficiaries for your account(s). • Set up 2-Factor Authentication security for your account.  NW Mutual/Dragoo Financial:  • Attend group educational seminars.	Points Awarded 2 points/event; maximum of 10 points allowed (points will not be awarded for duplicate events).

Eligibility Rules

• The incentive period begins on Jan. 1, 2025 and ends on Dec. 31, 2025.
• Employee must be enrolled as the subscriber on a Samaritan Choice medical plan as of Dec. 31, 2025 in order to be eligible to participate in the 2025 incentive.
• All points earned will be tracked online through the MyWellness portal, which can be accessed through MyHealthPlan.
• Points will automatically be uploaded into the MyWellness portal upon receiving a claim from your provider within 90 days of your preventive routine physical exam or preventive screening. Points for financial wellness events will be uploaded on a quarterly basis.
• Subscriber on a Samaritan Choice medical plan must complete at least 20 total points out of the possible 45 to qualify for the incentive for 2025. The $150 incentive will be distributed within the first two quarters of 2026.
• Service dates for eligible activities must be rendered within the incentive period and occur during time of employment with Samaritan Health Services.
• Services must be provided within the incentive period while the employee is enrolled as a subscriber on the Samaritan Choice medical plan.
• Employee must be an active employee at time of distribution in 2026.
• All self-reporting activities including dental exam and flu shot must be entered no later than Jan. 15, 2026. All activities recorded in January 2026 must have been completed by Dec. 31, 2025, to be eligible for the 2025 incentive.

How Do I Track My Points?

Track your points on the My Wellness portal (accessed from MyHealthPlan). 

Points are automatically uploaded within 90 days after receiving the claim from your provider for a preventive routine physical exam or preventive screening. Points for financial wellness events are uploaded quarterly. You must have at least 20 total points to qualify for the $150 Healthy Habits Incentive for 2025, which will be distributed within the first two quarters of 2026.

You are able to self-report points for your preventive dental exam and flu shot by logging into the MyWellness portal (accessed from MyHealthPlan).

When Will My 2025 Healthy Habits Incentive Be Distributed?

Your 2025 Healthy Habits Incentive will be taxable and distributed within the first two quarters of 2026. You must have at least 20 total points and be an active employee at time of distribution in 2026 to qualify for the $150 Healthy Habits Incentive.
