Alternate Log-in Instructions to Access the MyWellness Portal

If you are having trouble logging into the MyWellness portal or are not a current member, use these alternate log-in instructions.

Go to and follow the instructions below to set up your account and create a username and password.

  1. Enter employee ID (this will be your username):
    1. Employees: Use your 5-digit employee ID number.
    2. Spouses/domestic partners: Use the employee’s 5-digit ID number with an “S” added at the end plus the 2-digit suffix (e.g. 12345S02).
  2. Enter your birthday in a MMDDYYYY format (this will be your initial password).
  3. Click the “Log In” button.
  4. Read through and accept the “Terms and Conditions.”
  5. Optional: You can change your username or simply retype the one assigned.
  6. Create an eight-character password (password must include at least two numbers and one letter).
  7. Click the “Submit” button.
  8. Make sure to remember your username and password. You will need this to login in to the My Wellness portal and to take your online health assessment.
  9. Optional: Enter your mobile number or choose to uncheck the opt-in for text reminders.
  10. Click on “Proceed to Next Step”.
  11. Set your “Security Questions and Answers”.
  12. Click “Finish”.
