    If you had a service before you received your benefits card, your card did not work, or a service was not covered when you think it should have been, you can request reimbursement by completing this form: 

    Do you need to replace a benefit card that was lost, stolen or not received?

    • Go to MyHealthPlan.samhealth.org, log in and then click on the button that says, “Benefit Card Login.”
      Once in the portal, you can report the card sent as lost or stolen and a new card will be mailed to you. You may also contact Customer Service and request a new card be mailed.

    Frequently Asked Questions

    If you are a new member, you will receive your card within a few weeks of your completed enrollment. Your card will be mailed in a plain white envelope, so please watch your mail so it is not thrown away by mistake.

    Log in at myhealthplan.samhealth.org and click on Benefits Card login to view your balance. You can also check your balance 24/7 by calling 888-831-4668.

    If your card is lost or stolen, call Customer Service at 541-768-7866 or toll free at 866-768-7250 (TTY 800-735-2900 or 711) for help ordering a new card. The old card will be deactivated once a new card is ordered.

    For questions or assistance with your card, contact Customer Service at 541-768-4550 or toll free at 800-832-4580 (TTY 800-735-2900). Hours are 8 a.m. to 8 p.m. daily, Oct. 1 through March 31 and 8 a.m. to 8 p.m., Monday through Friday from April 1 through Sept. 30.
