Members will receive one benefit card for 2025. Your new card can be used to pay for certain health plan benefits and will also be loaded with the cash rewards earned from our Rewards & Incentives program.
Costco is currently not active in the new OTC network. An issue was discovered with their point-of-sale vendor on January 6th. We expected this issue would be resolved by January 21st, unfortunately, they have not been able to solve the problem. Currently there is not an estimate for when Costco will be able to join the network.
Our vendor partner is working diligently with Costco to resolve this issue as soon as possible. We apologize for any inconvenience this has caused.

As a member of Samaritan Advantage Health Plans, you have an over-the-counter (OTC) benefit every quarter. This benefit allows you to get OTC products you may need. Be sure to use your Samaritan Advantage Health Plans Benefits Mastercard when placing your order. Simply order online, mail your completed order form or call 800-752-3730 (TTY 711). Your order will be shipped directly to your door.

If you purchased an over-the-counter item before you received your benefits card, your card did not work, or an item was not covered when you think it should have been, you can request reimbursement by completing this form:
Do you need to replace a benefit card that was lost, stolen or not received?
If you are a new member, you will receive your card within a few weeks of your completed enrollment. Your card will be mailed in a plain white envelope, so please watch your mail so it is not thrown away by mistake.
Rewards from 2024 will be be moved to your new 2025 card. They will unavailable from Jan. 1 to Jan. 10, 2025, while we move the money over to your new card.
After Jan. 10 you can use your 2024 rewards until June 30, 2025.
Log in at myhealthplan.samhealth.org and click on Benefits Card login to view your balance. You can also check your balance 24/7 by calling 888-831-4668.
If your card is lost or stolen, call Customer Service at 541-768-7866 or toll free at 866-768-7250 (TTY 800-735-2900 or 711) for help ordering a new card. The old card will be deactivated once a new card is ordered.
