Your 2025 Benefits Mastercard® has been mailed. If you haven’t yet received it, you should soon. Watch your mail. It will be sent in a plain white envelope. Do not throw it away.
The 2025 debit card is white. And this year you will only receive one card, not two. The new white card can be used to pay for certain health plan benefits and it will also be loaded with the cash rewards earned from our Rewards & Incentives program.

As a member of Samaritan Advantage Health Plans, you have an over-the-counter (OTC) benefit every quarter. This benefit allows you to get OTC products you may need. Be sure to use your Samaritan Advantage Health Plans Benefits Mastercard when placing your order. Simply order online, mail your completed order form or call 800-752-3730 (TTY 711). Your order will be shipped directly to your door.

If you purchased an over-the-counter item before you received your benefits card, your card did not work, or an item was not covered when you think it should have been, you can request reimbursement by completing this form.

Do you need to replace a benefit card that was lost, stolen or not received?
Go to MyHealthPlan.samhealth.org, log in and then click on the button that says, “Benefit Card Login”. Once in the portal, you can report the first card sent as lost or stolen and a new card will be mailed to you. Another option is to contact Customer Service and request a new card be mailed.
If you are a new member, you will receive your card within a few weeks of your completed enrollment. Your card will be mailed in a plain white envelope, so please watch your mail so it is not thrown away by mistake.
Log in at myhealthplan.samhealth.org and click on Benefits Card login to view your balance. You can also check your balance 24/7 by calling 888-831-4668.
If your card is lost or stolen, call Customer Service at 541-768-7866 or toll free at 866-768-7250 (TTY 800-735-2900 or 711) for help ordering a new card. The old card will be deactivated once a new card is ordered.
