Getting Started

We are here to help you complete this process of enrolling with Samaritan Advantage Health Plan HMO. Once you have decided which plan you’d like to enroll in, review our enrollment instructions (English).

For 2027, Samaritan Health Plans will be offering the Dual Advantage Plan only. This plan is available to those who have both medical assistance from the State of Oregon and Medicare.

Choose How You’d Like to Enroll

1. Enroll over the phone.
   Just call 866-747-5267 (TTY 800-735-2900 or 711), Mon.–Fri, 8:30 a.m. to 8 p.m. One of our enrollment experts will assist you in completing your enrollment.
2. Mail, fax or email a completed enrollment form.
   Return your completed enrollment form however it’s convenient for you.
   • You can mail your form to:
     Samaritan Health Plans HMO, Attn: Enrollment Dept., PO Box M, Corvallis, OR 97339.
   • You can fax your form to:  541-768-9778, Attn: Enrollment Dept.
   • You can email your form to: [email protected].
3. Enroll with CMS.
   Medicare beneficiaries may also enroll in Samaritan Health plans through the CMS Medicare Online Enrollment Center located at www.medicare.gov or by calling 1-800-MEDICARE (1-800-633-4227). TTY users can call 877-486-2048, 24 hours a day, seven days a week.
