Biometric Screening & Health Assessment Incentive

Understand Your Health Risks

From June 1 through Aug. 31, 2024, Samaritan Health Services will be offering a free biometric screening and health assessment for employees. Participating will help you understand your risks for certain diseases and conditions and help you track progress toward your health goals.

Participation will also help you qualify for an incentive with enrollment in the Samaritan Choice PPO Plan for 2025. Employees must complete two steps by Aug. 31, 2024 to be eligible for the incentive.

Go to the MyWellness portal and complete the following steps:
Step 1:  Complete the online health assessment. 

Step 2:  Complete one of the three biometric screening options

• Option 1:  Schedule through the MyWellness portal and attend a biometric screening at one of our five Samaritan hospitals.
• Option 2:  Order an at-home test kit by Aug. 15, complete it and mail it with a postmark no later than Aug. 31.
• Option 3:  Upload a health care provider screening form. If you had labs performed within the past year (Sept. 1, 2023 through Aug. 31, 2024), submit your results with the screening form by email, mail, fax or upload to the MyWellness portal. Completed forms must be submitted by Aug. 31, 2024 to count for completion of the biometric screening.

Employees hired on or before June 1, 2024 who complete a biometric screening and online health assessment from June 1 to Aug. 31, 2024 (completed by 11:59 p.m. on Aug. 31) and enroll in the Samaritan Choice PPO Plan during open enrollment in November will qualify for the 2025 incentive.
