Choose How You’d Like to Enroll

We are here to help you complete this process of enrolling with Samaritan Advantage Health Plan HMO. Once you have decided which plan you’d like to enroll in, review our enrollment instructions (English).

Click the button below to view or download the enrollment form for 2025. Then complete your enrollment following any of the four options below:

1. Enroll over the phone.
   Just call 866-747-5267 (TTY 800-735-2900 or 711), Mon.–Fri, 8:30 a.m. to 5 p.m. One of our enrollment experts will assist you in completing your enrollment.
2. Mail, fax, email or drop-off a completed enrollment form.
   Return your completed enrollment form however it’s convenient for you.
   • You can mail your form to:
     Samaritan Health Plans HMO, Attn: Enrollment Dept., PO Box M, Corvallis, OR 97339.
   • You can fax your form to:  541-768-9778, Attn: Enrollment Dept.
   • You can email your form to: [email protected].
   • Or you can drop off your form in the drop box at the main entrance of our offices at:
     2300 NW Walnut Blvd., Corvallis.
3. Enroll with CMS.
   Medicare beneficiaries may also enroll in Samaritan Health plans through the CMS Medicare Online Enrollment Center located at www.medicare.gov or by calling 1-800-MEDICARE (1-800-633-4227). TTY users can call 877-486-2048, 24 hours a day, seven days a week.
4. Enroll with an insurance agent.
   Samaritan partners with licensed, knowledgeable insurance agents who can help you select a plan that’s right for you.
