Choose How You’d Like to Enroll

We are here to help you complete this process of enrolling with Samaritan Advantage Health Plan HMO. Once you have decided which plan you’d like to enroll in, review our enrollment instructions and complete your enrollment any of the following ways:

1. Enroll over the phone.
   Just call 866-747-5267 (TTY 800-735-2900), Mon.–Fri, 8:30 a.m. to 5 p.m. One of our enrollment experts will assist you in completing your enrollment.
2. Enroll by mail, fax, email or drop-off.
   Simply complete our 2023 enrollment form and choose what’s convenient for you.
   
   You can mail your form to:
   Samaritan Health Plans HMO, Attn: Enrollment Dept., PO Box M, Corvallis, OR 97339.
   
   You may fax your form to: 541-768-9778, Attn: Enrollment Dept.
   
   You may email your form to:  SHPOMemberEnrollment@samhealth.org.
   
   Or you can drop your form in the drop box at the main entrance of our offices at 2300 NW Walnut Blvd., Corvallis.
3. Enroll with CMS.
   Medicare beneficiaries may also enroll in Samaritan Health plans through the CMS Medicare Online Enrollment Center located at www.medicare.gov or by calling 1-800-MEDICARE (1-800-633-4227). TTY users can call 877-486-2048, 24 hours a day, seven days a week.
