Billing Notice

April premiums can be paid by April 30, 2024, without penalty.
For our new and existing members, we do not terminate coverage for lack of payment on the first month. For those enrolled in auto pay, the next payment processing date is April 5. Please allow a couple of days for the payment to show up on your bank account or credit card transaction activity.

Your Membership Cards & How to Use Them

When you join Samaritan Health Plans, you will receive two cards — your Member ID card and a Benefits Mastercard. Advantage members can also earn an additional Rewards & Incentives Mastercard for completing preventive services. Let’s explore each of these cards further:

Your Member ID Card

Keep your Member ID card with you and present it when seeking medical, dental, vision, hearing or prescription services. It includes your member ID number and plan name.

Your Benefits Mastercard

Use this prepaid debit card to pay for certain dental, vision, hearing and over-the-counter items. You’ll receive this card automatically upon joining our plan.

 

Earn a Bonus Card — The Rewards & Incentives Mastercard

Earn this card for seeing your primary care provider and staying current on preventive health services. As a thank you for completing these services, you’ll receive this additional card. It looks similar to your Benefits Mastercard but is used differently. You can use this card at many retailers or to pay for other expenses.
