Access Provider Directories

From the lists below, select the plan you are enrolled in to ensure you find a provider that is covered by your plan.

  • If you are looking for Samaritan Medicare plan providers, select Samaritan Medicare Advantage.
  • If you are on the Oregon Health Plan, select InterCommunity Health Network CCO.
  • If you are an employee of Samaritan Health Services, select Samaritan Choice.
  • If your employer sponsors your health plan, select Employer Group.

Once you select your plan, you will see a Provider Search button to help you find in-network providers for your plan.

With a Samaritan Advantage plan, you have lots of options for getting the routine and specialty care you need. You’re free to use any of the doctors, specialists and hospitals that are part of our network, including those outside the Samaritan family.

It’s comforting to know that more than 90% of the doctors, hospitals and clinics in our area, and numerous specialists from Portland to Eugene, are all part of the Samaritan Health Plans network.

Samaritan Advantage Network Providers

Looking for a Dentist? 

Many dentists who have not opted out of Medicare will also accept Samaritan Advantage. If you need assistance locating a dental provider that has not opted out of Medicare, please call one of our Customer Service team members at 541-768-4550 (toll-free 800-832-4580 or TTY 800-735-2900) seven days a week from 8 a.m. to 8 p.m. to see if your dentist accepts Samaritan Advantage.

Get Care When You Travel

When you are away from the plan’s service area you may get care outside the service area, but your coverage is limited. The services we cover when you are outside our service area are:

  • Care for medical emergency (worldwide).
  • Urgently needed care (nationwide).
  • Renal dialysis.
  • Care that Samaritan Advantage Health Plans or a plan provider has approved in advance.

Out-of-Network Coverage

In general, beneficiaries must use network providers to access their medical benefits, except in non-routine circumstances requiring urgent or emergent care. A prior authorization is required to see a non-contracted provider. If you don’t have appropriate authorization from our plan before you see a provider that is not part of our plan’s network, you may have to pay for the services he/she provided you with.

We are continually looking for better ways to serve you. We know you will benefit from the convenience and in-network savings of a large nationwide network. As a Samaritan Choice member, you have access to three networks:

  • Samaritan Choice – Oregon.
  • First Choice – Alaska, Idaho, Montana, North Dakota, Oregon, South Dakota, Washington, Wyoming.
  • First Health – additional U.S. coverage outside of the First Choice network.

This plan pays for vision examinations, and corrective lenses and frames when prescribed by a licensed ophthalmologist or licensed optometrist, for you and your insured dependents. The Plan allows you to choose any licensed ophthalmologist, optician, or optometrist. However, for eye examinations, there is a difference in reimbursement for participating vision providers and non-participating vision providers.

Samaritan EPO Network

Tailored within Benton, Lincoln and Linn counties, this network is available to all employer groups enrolled in one of our EPO plans.

Samaritan PPO Network

This network includes providers throughout the state of Oregon.

 

First Choice Health Network

This network complements our PPO network in the State of Oregon.

Traveling or Residing Outside of Oregon

First Choice Health Network applies if traveling or residing in the following states: Washington, Alaska, Idaho, Montana, North Dakota, South Dakota and Wyoming.

First Health Network applies if traveling or residing in states outside of Oregon, Washington, Alaska, Idaho, Montana, North Dakota, South Dakota and Wyoming:

Alternative Care Network

The optional alternative care benefit provides coverage for naturopathic care, chiropractic services, acupuncture treatments and massage therapy through the CHP Group Network. Please note that provider information or plan participation status may change at any time.

Get Care From Anywhere

Teladoc is a telehealth service available every day of the year, 24 hours a day, for members. It provides on-demand health care, including video visits, from a website or mobile app.

Teladoc is an option to get more immediate care when you are out of town or your primary care provider may not be available. There is no copay or out-of-pocket costs for Teladoc visits.

Teladoc providers can diagnose, treat and prescribe medication for non-emergency conditions. These include treatments for the flu, sore throats, eye infections, bronchitis and much more.
