Your Membership Cards & How to Use Them

When you join Samaritan Health Plans, you will receive two cards — your Member ID card and a Benefits Mastercard. Advantage members can also earn an additional Rewards & Incentives Mastercard for completing preventive services. Let’s explore each of these cards further:

Your Member ID Card

Keep your Member ID card with you and present it when seeking medical, dental, vision, hearing or prescription services. It includes your member ID number and plan name.

Your Benefits Mastercard

Use this prepaid debit card to pay for certain dental, vision, hearing and over-the-counter items. You’ll receive this card automatically upon joining our plan.

 

Learn About the Rewards & Incentives Program

Earn cash rewards for seeing your primary care provider and staying current on preventive health services. These rewards will be loaded onto your Benefits Mastercard®.

Take advantage of this annual opportunity to have a one-on-one with your medical provider to discuss your health. Medicare fully covers your wellness visit.

The visit includes:

  • A review of your health history, health risks, medications and measurements.
  • Preventive care and advance care planning.
  • Personalized health advice.
